We share insights at Care Management Show

Community Integrated Care was proud to speak at the Care Management Show last week – one of the care sector’s most prominent events for leaders and decision-makers. Held at the NEC in Birmingham NEC on Friday 27th June, the show brought together providers, suppliers and experts for a day of learning, discussion and collaboration.

They're stood in front of a green, blue and purple background with 'Care Management Show' and a house logo on it.As part of the Home Care Theatre programme, Edd Terrey, Managing Director for our South, Central and Yorkshire regions, and Jen Moores, our Business Development Director, delivered a powerful session titled ‘From Hospital to Home: Working with commissioners to achieve best lives possible.’ Within their presentation, they shared a compelling case study of a young person with complex support needs who, after experiencing multiple placement breakdowns, is now thriving in her own home in the community.

Through this story and other case studies, they demonstrated how placing the individual’s needs and aspirations at the heart of the  transition process, while working closely with commissioners, family members, and key partners, can lead to exceptional outcomes. The session highlighted the importance of co-production, strong collaboration, and unwavering person-centred support.

Speaking after the event, Jen said, “It was brilliant to join the Care Management Show and to share all we’ve learnt with sector colleagues. The case studies we shared are a testament to what’s possible when we challenge traditional models of care, work together as a health and social care sector, and listen deeply to the people we support. True partnership with commissioners, a skilled, dedicated and supported team, and a commitment to personalised care has enabled us to support people across the UK to move from crisis to thriving, and that’s what great social care is all about.”
