Complex Care

Community Integrated Care is proud to offer a wide range of specialist and complex care and support to adults living with complex needs. It’s a vital aspect of our charity’s work that involves many different specialisms.

What is Complex Care?

Complex Care is a person-centered approach to meeting the needs of people with conditions that have a major impact on their lives. These sometimes include a combination of medical, mental health and social challenges.

Community Integrated Care supports people with life-changing conditions, injuries and illnesses such as:

  • Profound and multiple learning disabilities
  • Complex mental health conditions
  • Those who fall under the transforming care agenda
  • Acquired brain injury (ABI)
  • Major trauma
  • Orthopaedic trauma
  • Amputation
  • Spinal cord injury (SCI)
  • Congenital conditions

Our approach

Our goal is to ensure that anyone with Complex Care needs can live with dignity, independence and choice – whether they’ve had a condition since birth or have experienced a life-changing personal injury.

Our Person-Centred Care services focus on how people want to be supported, what they want to achieve in life and what’s important to them.

This personalisation is an essential part of what we do in supporting people to live the best lives possible.

Our team

Our highly skilled teams have the right experience and knowledge to support people with complex needs.

Each team member takes part in bespoke training programmes focused on the needs of each person supported, provided by our own dedicated in-house Learning & Development function.

How we can help

We always strive to deliver the highest quality support by providing consistent, long-term, reliable Complex Care. We recognise the demands of supporting people in their own homes and understand the sensitivity needed to maintain a comfortable, stable environment.

Our holistic approach to complex conditions takes in all the different aspects that contribute to excellent care. This includes, bespoke recruitment to ensure the people we support are well-matched with their teams, specialist training and ongoing support, practical support for our Commissioners and Case Managers combined with our in-house clinical and psychological expertise.
