We share expertise at Care Strategy Forum
Two senior leaders from Community Integrated Care took to the stage at last week’s Care Strategy Forum – an exclusive conference bringing together health and social care leaders from across the UK – to share their insights on some of the sector’s most pressing challenges and opportunities.
Samantha Brennan, Managing Director for our charity’s North West and North East regions, joined a panel of industry-leaders in specialist care, titled “Pathways to Independence – Supporting Transitions in Care” on Thursday 19th June. The session explored how care providers can play a central role in enabling smooth, person-centred transitions from hospital to home, especially for people with complex needs.
Samantha said, “Supporting people to move out of institutional settings to living with independence in their communities is one of the most important things we can do as a sector. It demands real collaboration across health and social care, creativity and innovation, and a deep understanding of the person’s unique goals and challenges. It’s at the heart of our charity and the very vision by which our organisation was founded.”

Joanna Scammell, our charity’s Managing Director for Technology Enabled Care, then delivered a thought-provoking session on Friday 20th June, day two of the conference. Her “Powered by People, Supported by Technology” session focused on how the right technology can be harnessed to enhance care delivery, improve outcomes, and support independence, all while keeping people’s lived experiences at the centre of innovation.
She shared practical steps for embedding meaningful innovation across services, discussed the benefits of integrated working across health, social care, and community support, and highlighted how personalised digital tools are transforming care planning and communication.
