Every single person we support has an outcome-focused support plan, which looks at how we can work together to turn their aspirations into reality. It might include goals around being healthy, more involved in the community, better connected with family and friends, or learning new skills.
We help people develop their own Recovery Toolkit, engage in therapy, establish positive daily routines, re-discover interests and work towards longer-term goals – essential for restoring hope and optimism. This means people can:
