Corrine’s Story: From hospital to home
Corrine is a person supported by Community Integrated Care in Clackmannanshire. The information captured in this case study has been retrieved through meaningful conversations with Corrine herself, and the people closest to her, such as family, care managers and her support team.
Corrine has a diagnosis of a learning disability, anxiety and autism, as well as multiple physical disabilities. She loves all things music and spends most of her day dancing and attending music classes. Corrine also enjoy arts, crafts and baking!
Corrine’s background
Growing up, Corrine’s behaviours that challenge became unmanageable for her family, and she was offered a place at a Child and Adolescent Inpatient Unit where she received professional care, and then placed within the Royal Scottish National Hospital, where for the next 20 years, she would continue to live in Hospital settings.
Finding a home with Community Integrated Care
Corrine’s transition to living in her own home began with her support team from Community Integrated Care visiting, and over time, the team learnt how to support her. Further on in Corrine’s transition period, she began visiting her home in Baingle Brae, supported by two members of her new support team. Her house was not yet fully furnished but she was able to get to know her team and get used to her new home! Corrine and her support team spent hours every day dancing, singing and getting creative. Due to living within the hospital environment for her entire adult life, Corrine had grown to know this as ‘home’, but the Community Integrated Care team supported her to discover that there’s a life outside the ward. She quickly learned to love her newfound freedom and ‘home’ became her beautiful bungalow in Tullibody.
Settling in
Corrine receives 10 hours of 2-1 support during the day and 1-1 in the evening and overnight. This works really well for her lifestyle as she loves to spend most of her time out and about, be that shopping or going to clubs. Having two members of Corrine’s team support her throughout the day ensures that she feels safe and fully supported, and her 1-1 time helps her to relax in the evenings. Since moving to Baingle Brae, her behaviours that challenge have decreased significantly and the frequency of Corrine requiring medications has also decreased because of this. She is supported effectively in a person-centred manner to de-escalate challenging feelings.
Life at Baingle Brae
It’s been a year since Corrine received the keys to her first ever home. She appears much happier, more relaxed and is adjusting to home life fantastically! She’s always keen to get involved in housework and can complete a lot of this independently. Corrine especially enjoys making sure all her laundry is done and washing the dishes. With support from her team, Corrine has decorated her home with things that she loves, including having a wall dedicated to her own art masterpieces. Since moving into her own home, Corrine has discovered a love of food and has been able to try so many new dishes. She’s known as Baingle Brae’s ‘Star Baker’ and is always making cakes for her neighbours.
Corrine’s life has changed massively since moving into her own home. She’s grown in confidence, learnt new skills, made new friends and feels so much happier. Her team have really taken the time to get to know her and share so many hobbies and interests. This has enabled them to provide Corrine with the best possible support so that she can truly live her best life possible.
