Physical Disabilities
Community Integrated Care provides a wide range of care and support for people with physical disabilities. We help people meet their health, personal and social needs. Most importantly, our focus is on maximising independence and enabling people to live fulfilling and happy lives.
Personalised Support
We are committed to delivering truly person-centred support. Therefore, we take the time to get to know each person, including how they want to be supported, what they want to achieve and what matters most to them. As a result, we can provide support that is tailored to their individual needs and aspirations.
Every person we support has an outcome-focused support plan. Together, we identify goals and the steps needed to achieve them. This helps us turn ambitions into reality and celebrate progress along the way.
Working in Partnership
Our highly trained nursing and support teams work closely with local health and social care professionals. For example, we collaborate with physiotherapists and occupational therapists to deliver high-quality support. Furthermore, this joined-up approach helps people achieve the best possible outcomes.
Assistive technologies and accessible environments
We have extensive experience in implementing assistive technologies to support the independence and safety of people with physical disabilities.
Community Integrated Care also has extensive links with housing associations, which means we are able to work in partnership to create accessible living environments that enable people with physical disabilities to thrive.
