Community Integrated Care has been delivering innovative services to people with learning disabilities since 1988. Today, as one of the UK’s largest and most successful social care charities, we provide specialist support across England and Scotland.

Personalised care and support

We are committed to delivering genuinely person-centred services. To achieve this, we focus on really getting to know the people we support, including how they want to be supported, their goals in life and what matters most to them. As a result, we can provide support that is as individual as they are.

Moreover, every person we support has an outcome-focused support plan that outlines how we can work together to turn aspirations into reality. We build a detailed picture of each person’s preferences, routines and ambitions. This means that our support reflects their unique needs and choices. Ultimately, our goal is to empower people to live with greater independence, pursue opportunities and achieve the best life possible.

From hospital to home

Corrine, who has a learning disability, is supported by Community Integrated Care in Clackmannanshire. After spending her entire adult life in a hospital setting, Corrine has found new freedom and independence outside the ward.

With the support of our team, she’s discovered a true sense of home and a life filled with possibility!

How we help

We support people with a range of learning disabilities, sometimes combined with other conditions such as autism and physical or sensory needs. This support is provided in a number of different ways:

A joined up approach

Our teams work closely with many other professionals, from Community Learning Disability Teams to Speech and Language Therapists and Occupational Therapists.

We have extensive experience of working with housing providers to develop exceptional supported living services and care homes too.

Led by the people we support

Our charity is committed to social inclusion. That’s why we believe it’s vital to continually listen and respond to the views of the people we support. We’ve created a wide range of opportunities for people to share their thoughts and experiences, so that we can make the most of valuable feedback. As a result, we can continually improve our services and ensure that the voices of the people we support help shape the way we work.

Person-centred reviews

We regularly host person-centred reviews with the people we support. During these reviews, we work closely with each person and the important people in their lives to understand what is working well and identify where improvements can be made. As a result, we can ensure support continues to reflect their needs, goals and aspirations while helping them achieve the best possible outcomes.

 

Quality Advisors

We have a fantastic team of Quality Advisors in every region where we work. These are people we support – trained and employed as ‘experts by experience.’ They give us valuable insights into our services from the perspectives of people who use them.

Positive Behavioural Support (PBS)

Our specialist Positive Behavioural Support (PBS) team works across all our regions, offering expert guidance to help colleagues deliver truly personalised care. In addition, they provide practical support and training to help teams achieve the best possible outcomes for the people they support.

We support teams in creating bespoke plans that help people better understand and manage behaviours that may challenge, while focusing on increasing choice, control and the voice of the people we support. As a result, people can play a more active role in decisions that affect their lives.

By working closely with our services, our PBS team helps build environments where people feel empowered, supported and able to live fulfilling lives. Ultimately, their expertise helps ensure support is tailored to each person’s needs, strengths and aspirations
