Every person we support has an outcome-focused support plan looking at how we can work together to make their aspirations a reality. We build a full picture of their preferences, routines and what they want to achieve.
Corrine, who has a learning disability, is supported by Community Integrated Care in Clackmannanshire. After spending her entire adult life in a hospital setting, Corrine has found new freedom and independence outside the ward.

We support people with a range of learning disabilities, sometimes combined with other conditions such as autism and physical or sensory needs. This support is provided in a number of different ways:
Our teams work closely with many other professionals, from Community Learning Disability Teams to Speech and Language Therapists and Occupational Therapists.
We have extensive experience of working with housing providers to develop exceptional supported living services and care homes too.
Our charity is committed to social inclusion – we believe it’s vital to continually listen and respond to the views of the people we support. We’ve created lots of opportunities to communicate so that we’re constantly making the most of valuable feedback.
We regularly host person-centred reviews with the people we support. These see us work with the person, and the important people in their lives, to look at what is working for them and what we can make better.
We have a fantastic team of Quality Advisors in every region where we work. These are people we support – trained and employed as ‘experts by experience.’ They give us valuable insights into our services from the perspectives of people who use them.
Our specialist Positive Behavioural Support (PBS) team works across all our regions, offering expert guidance to help colleagues deliver truly personalised care.
We support teams in creating bespoke plans that help people better understand and manage behaviours that may challenge, while focusing on increasing choice, control, and the voice of the people we support.
By working closely with our services, our PBS team helps build environments where people feel empowered, supported, and able to live fulfilling lives.
