Community Integrated Care leads some of the UK’s most ambitious and innovative programmes to tackle health inequalities – empowering people who draw on social care to live healthier, happier and more connected lives.

That’s why we’re the Great British Care Awards Care Innovators of the Year.

Why This Matters

Our audiences face significant health and inclusion challenges:

  • Disabled people are 4x more likely to feel lonely
  • People with learning disabilities face a 20-year life expectancy gap
  • 50% of autistic adults experience depression
  • Nearly half of adults with learning disabilities are inactive and at higher risk of obesity

As a charity delivering 8 million hours of care annually, we see these inequalities firsthand – and we’re committed to changing them through insight-led, inclusive, and uplifting programmes.

Our Approach

We lead partnerships and campaigns that promote:

  • Research and innovation in health and care

We design and deliver health and wellbeing programmes that are inclusive, evidence-based, and life-enhancing. These initiatives are shaped by the real experiences of the people we support and the insights we gain from delivering 8 million hours of care each year.

Active Lives, Healthier Futures

We have designed groundbreaking solutions for sport and physical activity, which empower health and dreams. We are a key partner of Sport England and UK Sport, and leading sporting bodies such as Rugby Football League, Lawn Tennis Association, Table Tennis England and British Cycling – developing physical activity solutions that push boundaries for inclusion and opportunity. Our work has been named as the ‘Best Community Scheme’ at the Sports Business Awards.

This includes:

  • The Community Integrated Care Learning Disability Super League – Our non-competitive variant of Rugby League enables people to live their dreams, playing for Rugby League’s biggest clubs at the sport’s biggest games. It achieves £1,250,000 of social value every year.
  • Care To Play – We’ve enabled thousands of people to get active at home with CareToPlay.co.uk, our training for accessible at home table tennis. This proudly won the Pride of Table Tennis Award.
  • Walk With Nature – We’ve partnered with RSPB, WWF and National Trust to create a sensory walk programme that brings comfort and connection. This was named as Scotland’s Innovation of the Year at the Great British Care Awards.

Tackling Isolation, Building Purpose

Health is underpinned by connection, meaning, and opportunity. Our programmes that reduce loneliness and empower people to live fuller, more independent lives. This includes:

  • Inclusive Volunteering™ – Delivered at major events like Rugby League World Cup, Tour Of Britain and UEFA Women’s Euro 2022, this model offers empowering personal development that builds skills, confidence, and connection.
  • What-To-Do.co.uk –Community Integrated Care’s groundbreaking digital platform bring free, live, and inclusive experiences to people who draw on social care.
    From Broadway stars and Strictly dancers to wellbeing workshops, music sessions, and creative classes, it empowers skills, connection and independence.
  • www.CareToCook.co.uk – Our campaign to empower people to develop a passion for cookery, championing low-cost, healthy and accessible meals.

Driving System Change Through Research and Innovation

We work with NHS bodies, universities, and system leaders to explore new ways to improve health outcomes in social care.

  • Hosting national campaigns on key issues like annual health checks and physical activity
  • Supporting academic research into health inequalitiesmental health, and inclusive care models
  • Sharing insights that influence policy, practice, and public understanding

We’re proud to share influence at an international level, including presenting at the SportInnovator Summit at the 2024 Paralympic Games.
