Finalists for three Complex Care Awards!

Community Integrated Care is proud to have been shortlisted for three prestigious accolades at the inaugural Neurological and Complex Care Awards, hosted by Care Talk Magazine. 

The awards celebrate excellence in neurological and complex care, recognising outstanding contributions across the health and social care sector. They highlight the positive impact of services supporting people living with neurological conditions, complex care needs and those on the neuro-rehabilitation pathway. 

We’re delighted to have three finalists: 

• Rachel Jones, Care and Support Worker of the Year Award  

Advanced Support Worker, Rachel, has been shortlisted for her outstanding support in enabling a person to transition from long-term hospital care into his new home. Through her dedication, patience and person-centred approach, Rachel built a trusted relationship and provided reassurance, making a significant life change into a successful experience. Rachel is a passionate advocate who ensures the people she supports are listened to and empowered to make their own choices.  

• Community Integrated Care, HOPE(S) Team and partners, Best Multidisciplinary Team Award  

One of our North West teams have been recognised for their collaborative work in supporting a person with complex care needs to move from a long-term hospital setting into their own home. Their shared commitment to person-centred support, has helped achieve life-changing outcomes, enabling the individual to settle into their community, achieve personal goals and live a more independent life. 

• Highlands Road, Outstanding Complex Care Provider 

Highlands Road has been shortlisted for its outstanding person-centred support of an individual with complex needs. With skilled, compassionate care, unwavering dedication and close collaboration with family, healthcare professionals and commissioners, the team has helped achieve life-changing outcomes. As a result, the person has gained greater independence, rebuilt important relationships, developed meaningful friendships, become more involved in their community and grown in confidence when making their own choices. Milestones that once seemed out of reach have now become a reality. 

Winners will be announced at the Neurological and Complex Care Awards gala event on 27 October 2026, and we wish all finalists the very best of luck. We are incredibly proud to be recognised and look forward to celebrating alongside our peers from across the sector.
