We Win National Award For Transforming Lives

Community Integrated Care and its partners have won a national award for supporting a person with complex needs to leave long-term hospital care and build a life in their own home.

Our team, working in partnership with Lets for Life, NHS Cheshire and Merseyside Integrated Care Board, and Liverpool City Council, was named Multidisciplinary Team of the Year at the Neurological and Complex Care Awards in London on Tuesday 27th October.

The award recognises teams that bring together expertise from across health and social care to achieve outstanding outcomes for people with complex care needs.

The partnership was recognised for helping a person move from a restrictive hospital setting into a home of their own, creating a bespoke support model centred around their individual needs, goals and aspirations.

Achieving this outcome required months of planning, collaboration and preparation. Our colleagues spent six months working alongside the previous provider, building trusted relationships and developing a deep understanding of how best to support the person to feel safe, confident and settled.

Today, the person is enjoying greater independence, making more everyday choices, developing relationships and spending more time in their community.

Throughout the journey, the team has worked closely with the person’s family and specialist advocate to ensure their voice, wishes and ambitions remain at the heart of their support.

In recognising the team’s achievement, the award judges praised their determination to overcome barriers and create opportunities for people with complex needs. They said: “Where others may see barriers, they see possibility. Through innovative, individualised solutions, relentless advocacy and an unwavering refusal to give up, the team creates opportunities once thought impossible.” 

Richard Simpson, Service Manager at Community Integrated Care, added: “It’s an amazing testament to the team and how everyone has pulled together. We always keep the people we support at the forefront of what we do, and that shared focus has made such a difference.”
